Instructions:

 

  • Fill out the enrollment form for each student or use the form below and return it to the Director by dropping it off at the school or scanning it with a signature and emailing it to director@sullivanmontessori.org.
  • Five weekly schedule options are available, except for infants.
  • A non-refundable registration fee of $100 per child is due with the enrollment form.
  • The enrollment application will not be considered until the enrollment form, the parent handbook acknowledgement form (last page of Parent Handbook), and application fee have all been received.
  • Additional Forms: The child’s most current vaccination record, medical examination form, state enrollment forms, permission to publish consent form, and feeding care plan (infants and one-year-olds only) must be turned in on or before the student’s start date.
  • Tuition rates for 2025-2026 are based on the student’s age on 08-01-2025.

  • Schedule Options (per child per week):
  • 5 days per week
  • 3 days per week: M/W/F or T/R/F
  • Tuition Rates (per child per Month):
  • Infant – Under 1: $695 (N/A for other weekly schedule options)
  • T1 : $665 for 5 days, $465 for 3 days
  • T2: $625 for 5 days, $435 for 3 days
  • Connection/T3: $590 for 5 days,  $415 for 3 days
  •  Preschool: $580 for 5 days, $410 for 3 days
  • Before & After Care Options  with monthly rates (per child per month):
  • Before Care (7:00 AM Drop-off): $50 for 5 days, $30 for 3 days
  • After Care (4:00 PM Pick-up): $50 for 5 days, $30 for 3 days
  • After Care (5:00 PM Pick-up): $65 for 5 days, $50 for 3 days
  • After  Care (5:30 PM Pick-up): $80 for 5 days, $65 for 3 days
